Bu strateji, arteriyel hastal\u0131\u011f\u0131n ilerlemesi s\u0131ras\u0131nda ortaya \u00e7\u0131kan NV&#8217;nin hemodinamik ve h\u00fccresel engellerini a\u015fmak i\u00e7in tasarland\u0131.  <a href=\"https:\/\/doi.org\/10.1002\/term.3284\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">https:\/\/doi.org\/10.1002\/term.3284<\/a><\/p>\n\n\n\n<p>NV, do\u011fan\u0131n arteriyel t\u0131kay\u0131c\u0131 hastal\u0131\u011f\u0131n kaudalindeki iskemik dokuya kan ak\u0131\u015f\u0131n\u0131 iyile\u015ftiren normal mekanizmad\u0131r. Vask\u00fcler biyologlar ve hematologlar, bu mekanizman\u0131n bir\u00e7ok unsurunu ayd\u0131nlatm\u0131\u015ft\u0131r. T\u0131kal\u0131 bir b\u00fcy\u00fck arterin (ana cadde) \u00e7evresinde a\u011f olu\u015fturan arter dallar\u0131n\u0131n (yan sokaklar) l\u00fcmen y\u00fczeyini kaplayan endotel h\u00fccreleri \u00fczerindeki kesme geriliminin artmas\u0131yla ba\u015flar.  <\/p>\n\n\n\n<p><strong>Anjiyogenez<\/strong>, iskemik dokudaki h\u00fccrelere oksijenli besleyici kan ak\u0131\u015f\u0131n\u0131 sa\u011flayan arteriyoller ve k\u0131lcal damarlar\u0131n ve metabolizman\u0131n at\u0131k \u00fcr\u00fcnlerini temizleyen ven\u00fcllerin hipoksiye ba\u011fl\u0131 b\u00fcy\u00fcmesidir. Anjiyografik olarak, arteriyogenez &#8220;tirbu\u015fon&#8221; \u015feklindeki kollateral arterler olarak g\u00f6r\u00fcnt\u00fclenir ve anjiyogenez dokuda k\u0131zar\u0131kl\u0131k olarak g\u00f6r\u00fcn\u00fcr. NV&#8217;yi te\u015fvik etmek i\u00e7in bug\u00fcne kadar tasarlanan stratejilerin bir eksikli\u011fi, \u00f6ncelikle iskemik biyo\u00e7evrenin iyile\u015ftirilememesidir.  <\/p>\n\n\n\n<p>Ayr\u0131ca, bu stratejiler, arteriyogeneze yol a\u00e7an endotel h\u00fccre aktivasyonunu ba\u015flatmak i\u00e7in gerekli olan ciddi \u015fekilde zay\u0131flam\u0131\u015f endotel kesme gerilmesini ele almam\u0131\u015ft\u0131r. Bu, kollateral arter ak\u0131\u015f\u0131n\u0131 ciddi \u015fekilde zay\u0131flatan \u00e7ok seviyeli arteriyel t\u0131kay\u0131c\u0131 hastal\u0131\u011f\u0131 olan hastalarda \u00f6zellikle \u00f6nemlidir.  <\/p>\n\n\n\n<p>Ek olarak, iskemik dokudaki stres alt\u0131ndaki h\u00fccreler, v\u00fccuttaki progenit\u00f6r h\u00fccrelerin ni\u015flerine ven\u00f6z d\u00f6n\u00fc\u015f yoluyla ta\u015f\u0131narak bu yararl\u0131 h\u00fccreleri aktive edilmi\u015f endotelyuma geri \u00e7a\u011f\u0131rmak i\u00e7in \u00f6nemli sinyal proteinleri (\u00f6rn. SDF-1\u03b1) salg\u0131lar.<\/p>\n\n\n\n<p>K\u00f6t\u00fc kan ak\u0131\u015f\u0131 sadece sinyal protein sentezini ve yay\u0131lmas\u0131n\u0131 bozmakla kalmaz, ayn\u0131 zamanda harekete ge\u00e7irilen h\u00fccrelerin hedef dokuya geri ta\u015f\u0131nmas\u0131n\u0131 da zay\u0131flat\u0131r. Kronik uzuv tehdit edici iskemi (CLTI) hastalar\u0131nda, t\u00fcm bu engelleri ortadan kald\u0131rmak i\u00e7in bald\u0131r, ayak bile\u011fi ve aya\u011fa d\u0131\u015far\u0131dan pulsatil mekanik kuvvet uygulayan bir cihaz kulland\u0131k (ArtAssist cihaz\u0131, ACI Medical (San Marcos, CA)). <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"stem-cell-mobilization-with-g-csf-boosting-blood-flow-naturally\">G-CSF ile K\u00f6k H\u00fccre Mobilizasyonu: Kan Ak\u0131\u015f\u0131n\u0131 Do\u011fal Yollarla Art\u0131rma<\/h2>\n\n\n\n<p>NV, b\u00fcy\u00fck \u00f6l\u00e7\u00fcde progenit\u00f6r h\u00fccre hatt\u0131ndan ve enflamatuar h\u00fccrelerden (monositler gibi) t\u00fcretilen h\u00fccresel unsurlar taraf\u0131ndan d\u00fczenlenir. <\/p>\n\n\n\n<p>\u0130leri ya\u015f, diyabet ve \u015fiddetli vask\u00fcler hastal\u0131\u011f\u0131 olan hastalarda, NV&#8217;nin \u00f6n\u00fcndeki en b\u00fcy\u00fck engel, dola\u015f\u0131mdaki progenit\u00f6r h\u00fccrelerin say\u0131s\u0131n\u0131n ve i\u015flevinin azalmas\u0131d\u0131r. Bu nedenle, yararl\u0131 progenit\u00f6r h\u00fccrelerin ve monositlerin kullan\u0131labilirli\u011fini de iyile\u015ftiremedik\u00e7e, biyosentetik mikro ortam\u0131 iyile\u015ftirmek NV&#8217;yi iyile\u015ftirmek i\u00e7in tek ba\u015f\u0131na yeterli de\u011fildir.  <\/p>\n\n\n\n<p>Bu engeli, k\u00f6k h\u00fccre tedavisinde hematopoietik ve progenit\u00f6r h\u00fccre mobilizasyonunu ve ba\u011f\u0131\u015f\u0131kl\u0131k sisteminin g\u00fc\u00e7lendirilmesini sa\u011flamak i\u00e7in yayg\u0131n olarak kullan\u0131lan Gran\u00fclosit Koloni Stim\u00fclan Fakt\u00f6r (G-CSF) ile a\u015fmaya karar verdik. ArtAssist cihaz\u0131n\u0131 g\u00fcnde en az 3 saat (otururken) takan CLTI hastalar\u0131na, 72 saatte bir 10 doz olmak \u00fczere yeni bir dozajda (7-10 mcg\/kg) Neupogen (amgen Inc) kulland\u0131k. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"limb-salvage-achieved-in-no-option-patients\">Se\u00e7enek Kalmayan Hastalarda Uzuv Kurtarma Ba\u015far\u0131s\u0131<\/h2>\n\n\n\n<p>Tedavi edilen CLTI hastalar\u0131, geleneksel revask\u00fclarizasyon se\u00e7enekleri (cerrahi baypas, anjiyoplasti, aterektomi, lazerle yeniden kanalla\u015ft\u0131rma) t\u00fckenmi\u015f olan &#8220;se\u00e7enek kalmam\u0131\u015f&#8221; CLTI hastalar\u0131 olarak de\u011ferlendirildi. <\/p>\n\n\n\n<p>B\u00fcy\u00fck amputasyon yerine tedavi edildiler. \u0130lk tedavi edilen hastalarda uzuv kurtarma ve iskemik yaralar\u0131n iyile\u015fmesi, anjiyografik olarak arteriyogenez, anjiyogenez ve en \u015fa\u015f\u0131rt\u0131c\u0131 \u015fekilde, daha \u00f6nce belgelenmi\u015f segmental arteriyel okl\u00fczyonun rekanalizasyonunun kan\u0131tlanmas\u0131na yol a\u00e7t\u0131.  <\/p>\n\n\n\n<p>Bu, plazmin (fibrinolizi tetikleyen) ve fibrin y\u0131k\u0131m \u00fcr\u00fcnleri (fibrinolizi ge\u00e7iren tromb\u00fcs taraf\u0131ndan \u00fcretilen) ile bir dizi pro-anjiyojenik proteinin plazma d\u00fczeylerinin \u00f6l\u00e7\u00fclmesine yol a\u00e7t\u0131. Bu Enzim Ba\u011fl\u0131 \u0130mm\u00fcnosorbant Testleri (ELISA), k\u00f6k h\u00fccre destekli yakla\u015f\u0131m\u0131m\u0131z\u0131n ger\u00e7ekten NV&#8217;ye yol a\u00e7t\u0131\u011f\u0131n\u0131 ve NV&#8217;nin \u00f6n\u00fcndeki bir ba\u015fka b\u00fcy\u00fck engeli, yani kronik trombu (<a href=\"https:\/\/doi.org\/10.1002\/term.3284\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">https:\/\/doi.org\/10.1002\/term.3284<\/a>) a\u015ft\u0131\u011f\u0131n\u0131 do\u011frulad\u0131. <\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button has-custom-width wp-block-button__width-100\"><a class=\"wp-block-button__link wp-element-button\" href=\"#requestform\">\u00dccretsiz dan\u0131\u015fmanl\u0131k al\u0131n<\/a><\/div>\n<\/div>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"clinical-and-molecular-evidence-of-neovascularization\">Neovask\u00fclarizasyonun Klinik ve Molek\u00fcler Kan\u0131tlar\u0131<\/h2>\n\n\n\n<p>NV&#8217;de rol oynayan h\u00fccresel ve molek\u00fcler unsurlar\u0131n ve bunlar\u0131n bu umut verici NV stratejisiyle ilgisinin ayr\u0131nt\u0131l\u0131 bir incelemesi <a href=\"https:\/\/doi.org\/10.1016\/j.jvsvi.2025.100190\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">https:\/\/doi.org\/10.1016\/j.jvsvi.2025 adresinde bulunabilir. 100190<\/a> adresinde, h\u00fccresel kat\u0131l\u0131mc\u0131lar\u0131n her birinin (endotel, progenit\u00f6r h\u00fccreler, n\u00f6trofiller, monositler vb.) hastalar\u0131m\u0131z \u00fczerinde ger\u00e7ekle\u015ftirdi\u011fimiz ayr\u0131nt\u0131l\u0131 molek\u00fcler ve sitometri testlerinden elde edilen g\u00f6zlemleri a\u00e7\u0131klamaya nas\u0131l katk\u0131da bulunduklar\u0131 vurgulanmaktad\u0131r. <\/p>\n\n\n\n<p>Bu veriler, kan ak\u0131\u015f\u0131n\u0131n iyile\u015fti\u011fine dair hemodinamik ve anjiyografik kan\u0131tlar\u0131 destekledi ve h\u00fccre tedavisinin molek\u00fcler etkisini \u00f6l\u00e7menin (\u00f6nceki NV klinik denemelerinde eksik olan) ne kadar \u00f6nemli oldu\u011funu vurgulad\u0131. <\/p>\n\n\n\n<p>&#8220;Se\u00e7enek olmayan&#8221; CLTI hastalar\u0131, zorlu ko\u015fullara ra\u011fmen uzuvlar\u0131n\u0131 kurtarmay\u0131 ba\u015fard\u0131lar. Ancak, tedavimizin kan ak\u0131\u015f\u0131n\u0131 iyile\u015ftirme h\u0131z\u0131 ile ileri d\u00fczey vask\u00fcler hastal\u0131\u011f\u0131n ilerleme h\u0131z\u0131 aras\u0131nda a\u00e7\u0131k\u00e7a bir yar\u0131\u015f s\u00f6z konusudur.   <\/p>\n\n\n\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/?term=limb+salvage\" target=\"_blank\" rel=\"noreferrer noopener\">uzuv kurtarma<\/a> kan ak\u0131\u015f\u0131n\u0131n iyile\u015ftirilmesine (stratejimizin amac\u0131) ba\u011fl\u0131 olmakla birlikte, ayn\u0131 zamanda ileri d\u00fczeyde ve \u00f6zenli yara bak\u0131m\u0131, sepsis y\u00f6netimi, debridman, beslenme, komorbiditeler, uyum ve travman\u0131n \u00f6nlenmesine de ba\u011fl\u0131d\u0131r. <\/p>\n\n\n\n<p>Bu nedenle, NV stratejisinin uygulanmas\u0131, kronik iskeminin erken a\u015famalar\u0131nda, geleneksel m\u00fcdahaleler yap\u0131lmadan ve iskemik yaralar sepsis nedeniyle kapsaml\u0131 debridman gerektirecek kadar a\u011f\u0131rla\u015fmadan \u00f6nce en uygunudur. Geleneksel y\u00f6ntemlerle revask\u00fclarizasyon veya zorunlu cerrahi yara tedavisi i\u00e7in ne kadar \u00e7ok ameliyat yap\u0131l\u0131rsa, anatomi o kadar fazla de\u011fi\u015fir.  <\/p>\n\n\n\n<p>Akut inflamasyon ve postoperatif fibroz, NV i\u00e7in potansiyel engellerdir.
