Ils sont lib\u00e9r\u00e9s naturellement par la plupart des cellules de l&rsquo;organisme, en particulier celles qui participent \u00e0 la croissance, \u00e0 la r\u00e9paration ou \u00e0 la r\u00e9ponse immunitaire. Il faut les consid\u00e9rer comme de petits courriers &#8211; des colis biologiques envelopp\u00e9s dans une membrane protectrice &#8211; transportant une cargaison pr\u00e9cieuse et hautement sp\u00e9cifique de prot\u00e9ines, d&rsquo;instructions g\u00e9n\u00e9tiques (sous la forme d&rsquo;ARNm et de microARN), d&rsquo;enzymes et parfois m\u00eame d&rsquo;hormones.  <\/p>\n\n<p>Chaque exosome refl\u00e8te l&rsquo;identit\u00e9 et la sant\u00e9 de la cellule dont il provient. Si une cellule souche est jeune et dynamique, ses exosomes seront charg\u00e9s de messages favorisant la croissance et anti-inflammatoires. Si une cellule est endommag\u00e9e ou \u00e2g\u00e9e, ses exosomes peuvent v\u00e9hiculer le contraire.  <\/p>\n\n<p>These nanoscale, critical intercellular messengers carry biomolecular signatures reflecting their parent cells, making them powerful tools for non-invasive diagnostics and therapeutic monitoring. In a future section, we will talk about exosome-based drug delivery.<\/p>\n\n<p>C&rsquo;est pourquoi la source est importante. C&rsquo;est \u00e9galement la raison pour laquelle la th\u00e9rapie par exosomes, lorsqu&rsquo;elle est effectu\u00e9e correctement, est si prometteuse : nous n&rsquo;essayons pas de forcer l&rsquo;organisme \u00e0 gu\u00e9rir &#8211; nous lui transmettons les bons messages pour qu&rsquo;il puisse se gu\u00e9rir lui-m\u00eame. <\/p>\n\n<h2 class=\"wp-block-heading\" id=\"how-exosomes-work-beyond-the-microscope\">Fonctionnement des exosomes : au-del\u00e0 du microscope<\/h2>\n\n<p>En laboratoire, on peut observer les exosomes se lier aux cellules r\u00e9ceptrices et fusionner avec leurs membranes. Au microscope, tout est tr\u00e8s technique : fusion des v\u00e9sicules, liaison des r\u00e9cepteurs, traduction de l&rsquo;ARN. <\/p>\n\n<p>Mais dans le corps, ce qui se passe est beaucoup plus po\u00e9tique.<\/p>\n\n<p>Les exosomes ne se contentent pas de d\u00e9poser des produits chimiques. Ils donnent des instructions. Un neurone endommag\u00e9 par un accident vasculaire c\u00e9r\u00e9bral peut recevoir un exosome qui l&rsquo;aide \u00e0 r\u00e9sister \u00e0 l&rsquo;apoptose. Une cellule immunitaire qui s&rsquo;enfonce dans la spirale de l&rsquo;auto-immunit\u00e9 pourrait recevoir l&rsquo;ordre de se retirer. Une cellule cutan\u00e9e prise dans une inflammation chronique pourrait recevoir le message de se reconstruire et non de r\u00e9agir.    <\/p>\n\n<p>Ce type de signalisation est orchestr\u00e9 et non al\u00e9atoire. Et contrairement aux m\u00e9dicaments pharmaceutiques qui agissent sur plusieurs syst\u00e8mes \u00e0 la fois &#8211; parfois avec des effets n\u00e9fastes &#8211; les exosomes sont souvent cibl\u00e9s. Ils portent des \u00ab\u00a0codes postaux\u00a0\u00bb mol\u00e9culaires qui leur permettent d&rsquo;atteindre des tissus sp\u00e9cifiques.  <\/p>\n\n<p>Ils ne sont pas seulement petits. They don\u2019t build proteins\u2014they control which proteins get built. For this reason, exosomes do not alter the patient&rsquo;s genetic code or cause genetic transfer. They only contribute to regeneration by accelerating the synthesis of important proteins.<\/p>\n\n<p>Parmi les <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/?term=microRNA\" target=\"_blank\" rel=\"noreferrer noopener\">microARN<\/a>trouv\u00e9s dans les exosomes th\u00e9rapeutiques, on peut citer<\/p>\n\n<ul class=\"wp-block-list\">\n<li>miR-124 &#8211; Favorise la neurogen\u00e8se, calme l&rsquo;inflammation neuronale<\/li>\n\n\n\n<li>miR-21 &#8211; R\u00e9duit la fibrose, un facteur cl\u00e9 de l&rsquo;inflammation chronique<\/li>\n\n\n\n<li>miR-146a &#8211; Aide \u00e0 calmer l&rsquo;hyperactivit\u00e9 auto-immune<\/li>\n\n\n\n<li>miR-155 &#8211; Pro-inflammatoire ; souvent filtr\u00e9 dans les exosomes de qualit\u00e9 clinique<\/li>\n<\/ul>\n\n<p>Chez les patients auto-immuns, l&rsquo;\u00e9quilibre de ces miARN peut faire la diff\u00e9rence entre une fatigue chronique et un soulagement significatif.<\/p>\n\n<h3 class=\"wp-block-heading\" id=\"proteins-enzymes\">Prot\u00e9ines &amp; enzymes<\/h3>\n\n<p>Les exosomes sont souvent porteurs :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Cytokines (pour calmer l&rsquo;inflammation)<\/li>\n\n\n\n<li>Enzymes (pour d\u00e9composer le tissu cicatriciel ou remodeler les zones endommag\u00e9es)<\/li>\n\n\n\n<li>Prot\u00e9ines de choc thermique (pour r\u00e9duire le stress oxydatif)<\/li>\n\n\n\n<li>T\u00e9traspanines (marqueurs qui aident \u00e0 identifier et \u00e0 trier les exosomes)<\/li>\n<\/ul>\n\n<p>Chaque prot\u00e9ine joue un r\u00f4le dans l&rsquo;\u00e9laboration de la r\u00e9ponse. Il ne s&rsquo;agit pas de devinettes chimiques. Il s&rsquo;agit d&rsquo;une chor\u00e9graphie mol\u00e9culaire.  <\/p>\n\n<h3 class=\"wp-block-heading\" id=\"lipids\">Lipides<\/h3>\n\n<p>Il ne s&rsquo;agit pas seulement de membranes passives. Les lipides exosomaux tels que la sphingomy\u00e9line, la phosphatidyls\u00e9rine et les c\u00e9ramides influencent la signalisation cellulaire, la fluidit\u00e9 des membranes et m\u00eame le ciblage immunitaire. En dermatologie, par exemple, ces lipides aident les exosomes \u00e0 fusionner avec les k\u00e9ratinocytes et les fibroblastes pour am\u00e9liorer la r\u00e9paration de la peau.  <\/p>\n\n<h2 class=\"wp-block-heading\" id=\"why-exosome-source-matters-more-than-you-think\">Pourquoi la source de l&rsquo;exosome est plus importante que vous ne le pensez<\/h2>\n\n<p>Dans le monde clinique, je dis souvent aux patients : \u00ab\u00a0Le pouvoir des exosomes d\u00e9pend de leur origine\u00a0\u00bb.<\/p>\n\n<h3 class=\"wp-block-heading\" id=\"umbilical-cord-derived-msc-exosomes\">Exosomes de CSM d\u00e9riv\u00e9es du cordon ombilical<\/h3>\n\n<p>C&rsquo;est mon choix pr\u00e9f\u00e9r\u00e9 pour la plupart des applications r\u00e9g\u00e9n\u00e9ratives. Pourquoi ? Ils sont :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Jeunesse (pas de signaux d&rsquo;inflammation li\u00e9s \u00e0 l&rsquo;\u00e2ge)<\/li>\n\n\n\n<li>Immunologiquement neutre (faible risque de rejet)<\/li>\n\n\n\n<li>Riche en facteurs de croissance, en miARN et en mol\u00e9cules anti-fibrotiques<\/li>\n\n\n\n<li>Provenant de naissances saines et \u00e0 terme, d&rsquo;origine \u00e9thique<\/li>\n<\/ul>\n\n<h3 class=\"wp-block-heading\" id=\"placental-exosomes\">Exosomes placentaires<\/h3>\n\n<p>D&rsquo;une puissance similaire \u00e0 celle du cordon ombilical, avec un fort potentiel immunomodulateur. Elles sont souvent utilis\u00e9es chez les patients souffrant d&rsquo;inflammation syst\u00e9mique ou de maladies auto-immunes. <\/p>\n\n<h3 class=\"wp-block-heading\" id=\"adipose-or-bone-marrow-sources\">Sources adipeuses ou de moelle osseuse<\/h3>\n\n<p>Bien que cette technique soit encore utilis\u00e9e dans certaines cliniques, l&rsquo;\u00e2ge du donneur peut avoir une incidence consid\u00e9rable sur la qualit\u00e9 des exosomes. Chez les patients plus \u00e2g\u00e9s, le message peut \u00eatre moins ax\u00e9 sur la r\u00e9g\u00e9n\u00e9ration et plus sur la maintenance ou l&rsquo;inflammation. <\/p>\n\n<h3 class=\"wp-block-heading\" id=\"tumor-cell-exosomes\">Exosomes de cellules tumorales<\/h3>\n\n<p>Utilis\u00e9 exclusivement dans la recherche. Dangereux sur le plan th\u00e9rapeutique. Ils sont souvent porteurs de signaux oncog\u00e8nes (favorisant le cancer).  <\/p>\n\n<h2 class=\"wp-block-heading\" id=\"from-cell-to-vial-the-exosome-manufacturing-journey\">De la cellule au flacon : Le parcours de la fabrication des exosomes<\/h2>\n\n<p>La cr\u00e9ation d&rsquo;exosomes s\u00fbrs, puissants et coh\u00e9rents ne se fait pas dans une centrifugeuse d&rsquo;arri\u00e8re-boutique. A minor impurity can affect the therapeutic response &#8211; or worse, introduce a risk. That\u2019s why we don\u2019t cut corners.
